What to bring with you for your physical
- Please bring your driver's license or some form of government identification that has a picture included. Please also bring a list of any medications that you are currently taking.
- Proof of use of CPAP or BPAP machine.
- Prescription glasses/contacts
- Hearing aids
- If you had any major surgeries or diagnosis in the past year; a letter from your doctor stating that you can safely operate a commercial vehicle.
- A copy of any recent health scans (A1c, MRI, EKG, ECG, Stress Test, PSA levels)
